HOW DO I APPLY FOR A 2011-12 SCHOLARSHIP?
The Mayde Creek Athletic Booster Club (MCHSABC) will award scholarship awards. The funds will be paid directly to the student’s university after receiving proof of enrollment, and in accordance with NCAA policies.
The purpose of the scholarships will be to recognize MCHS athletes for their contribution to MCHS athletics based on their athletic achievements, leadership, character, and initiative.
REQUIREMENTS FOR 2011-12 APPLICATION:
The student’s family must be an active member in good standing of the MCHSABC on or before December 31, 2011.
The applicant must:
1) Have maintained a high school cumulative GPA of 2.8 or higher to be eligible to apply.
2) The student must participate in a UIL approved sport at the VARSITY level during their senior year.
3) The student must have participated in the MCHS athletic program for one year prior to his/her senior year.
4) The application MUST be turned in to the Athletic Director-> LANCE CARTER, in his office, on or before Friday, March 30, 2012 2:35 p.m. CST.
THE APPLICANT MUST SUBMIT THE FOLLOWING TO THE ATHLETIC DIRECTOR:
1) The scholarship application properly filled out.
2) An unofficial transcript of all school credits.
3) Two letters of recommendation from non-family, responsible community, church or school professionals.
4) A one-page typewritten statement giving reasons why you desire this scholarship award and are worthy to be a recipient.
** To receive this one-time scholarship award, recipients must present their Fall 2011 college/university proof of enrollment by September 1, 2012 to receive compensation.
